Membership is for adults who value consistent, physician-led care within a private, small-practice model. I offer clear follow-through, well-defined expectations, and a relationship built on trust and respect. Follow-up visits, treatment planning, and medication management (when clinically appropriate) are all coordinated directly by me.
Membership does not include emergency services, unscheduled calls, or crisis care. All care decisions happen in scheduled visits or through documented asynchronous submissions. I do not see patients under 18, or outside New York State, and do not provide substance use or developmental disability specialty care.

Your ongoing care is carefully overseen with scheduled follow-up and optional asynchronous visits.
Included: Ongoing treatment planning and medication management if clinically appropriate, scheduled visits during practice hours, and the option for asynchronous visits via secure video. Administrative and scheduling support are handled by an answering service.
Not included: Emergency availability, unscheduled phone calls, crisis intervention, or care for minors, substance use, or developmental disabilities. All clinical interactions are scheduled and documented.
