2. Description of Services

Private Psych MD provides psychiatric evaluation, medication management, and related mental health services through in-person and telehealth appointments. Our services are provided by licensed psychiatric professionals in the state of New York. We operate a membership-based practice model for ongoing psychiatric care.

3. Eligibility

To use our services, you must be at least 18 years of age or have parental/guardian consent. You must provide accurate and complete information during registration and maintain the accuracy of such information. You must be located in a state where we are licensed to practice when receiving telehealth services.

5. Appointments and Cancellations

Appointments must be scheduled in advance through our designated scheduling system. We require at least 24 hours notice for cancellations or rescheduling. Late cancellations or missed appointments may be subject to a fee. We reserve the right to discharge patients who repeatedly miss appointments without notice.

6. Payment and Fees

Private Psych MD operates as a private-pay practice. Payment is due at the time of service unless other arrangements have been made. We may offer membership plans with specific terms and pricing. Membership fees are non-refundable unless otherwise specified. We do not bill insurance directly; however, we can provide documentation for you to submit to your insurance for potential reimbursement.

7. Telehealth Services

Telehealth services are subject to additional considerations. You must be in a private, secure location during telehealth appointments. You are responsible for ensuring adequate internet connectivity. Telehealth may not be appropriate for all conditions or situations. We reserve the right to require in-person visits when clinically necessary.

8. Patient Responsibilities

You agree to provide accurate and complete information about your health history, current symptoms, and medications. You will follow the treatment plan as discussed with your provider or communicate concerns promptly. You will notify us of any changes in your condition or if you experience adverse effects. You will not share, sell, or misuse any prescribed medications.

9. Intellectual Property

All content on our website, including text, graphics, logos, images, and software, is the property of Private Psych MD and is protected by copyright and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from our content without written permission.

10. Limitation of Liability

To the fullest extent permitted by law, Private Psych MD shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our website or services. Our total liability shall not exceed the amount paid by you for services in the twelve months preceding the claim.

These Terms shall be governed by and construed in accordance with the laws of the State of New York, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be resolved in the courts of New York.

14. Changes to Terms

We reserve the right to modify these Terms at any time. Changes will be effective immediately upon posting to our website. Your continued use of our services after any changes constitutes acceptance of the new Terms.
